SELF ASSESSMENT FORM

Yay! You have School Swimming coming up with Aquastar Swim Schools.

Please complete form and send it back to your school (not to Aquastar).

The Self Assessment form provides us with a rough idea of your swimmer's ability and assists us to organise students into groups for the first day of swimming.

If your self assessment is a bit off, no worries! A thorough in-pool swimming assessment will be conducted on the first day of the program to ensure all students are reorganised into the appropriate ability-based levels.
